This is the format:

For example: Kane#4701 [Ben]
If you don't know who you were banned by, which is a common occurrence with Discord bans, simply leave out the end and only submit it with the title having your Discord name.

If the staff member who banned you is no longer in our team, your appeal will be reassigned to another staff member or group.
