Have you ever been banned from any LS-RP services (including the Discord server)?: No.

 

Briefly explain why you need to use a VPN/proxy to play LS-RP: Given I use a VPN almost 24/7 when I'm not playing LSRP, I find it highly inconvenient to have to turn off my VPN whenever I want to play LS-RP. I regularly use one for privacy concerns and to avoid having my IP logged by various services (ex. IRC) that display them publicly to moderators or members.
